Clinical Outcomes of Submucosal Invasive Gastric Cancer After Endoscopic Submucosal Dissection: A Single-Center Study

Submucosal invasive gastric cancer (SMGC) presents a clinical challenge, particularly when detected in its early stages. Endoscopic Submucosal Dissection (ESD) has emerged as a promising treatment modality, offering a less invasive approach than traditional surgery while enabling en bloc resection and precise histological evaluation.

This blog explores the clinical outcomes of SMGC following ESD, as reported in a single-center retrospective study — and how such findings could influence global practices in gastrointestinal oncology.

What Is Submucosal Invasive Gastric Cancer?

SMGC is a form of early-stage gastric cancer that extends into the submucosal layer. Its invasive nature requires timely intervention to prevent lymphovascular spread. Traditionally, gastrectomy with lymph node dissection has been the treatment standard — but ESD is now proving to be a viable, organ-preserving alternative.

Why ESD Is a Game-Changer in SMGC

ESD enables complete removal of gastric lesions with minimal disruption to surrounding tissue. This technique provides:

·         High en bloc and R0 resection rates

·         Accurate histopathological assessment

·         Lower complication and recurrence rates when performed by skilled endoscopists

In this single-center study, patients who underwent ESD for SMGC demonstrated excellent local control and comparable long-term outcomes to those treated surgically.

Study Highlights: Clinical Outcomes at a Glance

The retrospective analysis found:

·         En bloc resection rate: 95%

·         R0 resection rate: 89%

·         Local recurrence rate: <2%

·         Overall survival: Comparable to surgery in patients without high-risk lymphovascular invasion

These results underscore the growing relevance of ESD in treating select cases of SMGC, especially in centers with high procedural expertise.
